New Clients
New clients are requested to arrive at least 15 minutes prior to their appointment to complete necessary ‘new patient questionnaire’ forms.
If you are running late, we appreciate a simple phone call to notify us. We may need to rearrange your appointment to accommodate other patients.
Cancellation Policy
If you need to cancel or reschedule your appointment, a minimum of 48 hours’ notice is required. You can cancel by phone or email.
Appointments canceled with less than 48 hours’ notice will forfeit the deposit paid.
Failure to attend an appointment without notice (“no show”) will result in the loss of the deposit.
